Preparing for Your Visit
Due to COVID-19 and until further notice, we are not allowing patients and/or guests in our waiting room. Please do not bring anyone with you to your appointment, unless approved in advance. When you arrive, please call us from your car at 904-273-5454 and we will come out to you to escort you in at your appointment time.
Cancellations must be made 48 hours in advance of the scheduled appointment time. Otherwise, you will be subject to a non-refundable fee.
Late arrivals may result in an abbreviated appointment time.
Failure to show for appointment will result in a non-refundable fee.
We will collect a non-refundable $100 in advance for all patient appointments. It will be used toward your treatment or any products/services that we offer. If you do not show for your scheduled appointment or late cancel(less than 48 hours prior to appointment) it will be used to pay that fee.
Upon entering the practice, we kindly ask that you silence your mobile device(s) and refrain from using your mobile phone out of respect for other guests.
